The Sympathetic Dominance Problem

Why no protocol works until you fix this first
Darin J. Mitchell, DC · Natural Health Houston
Every practitioner has this patient on the schedule this week. They eat clean, take professional-grade supplements, and have had every test. And they are not better, often worse with each thing we suggest or add.
Most of these patients aren't treatment-resistant. They're sequence-injured. We're running good protocols on a nervous system stuck in threat mode, and a nervous system in threat mode can't use what we give it.
Two doors, same room
Years ago I had a patient I could not help. Three years of severe back pain and a spine so tight I could not adjust it. MRI, injections, decompression, laser, CBD: nothing changed. During COVID he came back pain-free. Someone else had handed him John Sarno's Healing Back Pain, and he'd started asking, "What if this isn't physical?" The pain moved to his foot, then left, and the spine I couldn't move for three years “opened up” under my hands.
I had been working the same problem from the other end. Convinced I had leaky gut and IBS-D, I was taking high-dose glutamine. At a seminar in St. Louis I tried an alpha-wave stimulation device during a lecture and came away disoriented. I didn't sleep well that night, and the next morning, in the same seat on the same plane I'd flown in on, I had my first panic attack.
It took ten years of "me-search" to understand. I carry a SNP in GAD, the enzyme that converts glutamate to GABA, plus the methylation and micronutrient gaps that ride with it. I had loaded the system with glutamine, the precursor to glutamate, given it an electrical push, and had no bridge to GABA. Accelerator floored, no brakes.
I refused benzodiazepines and SSRIs. Prescription sleep medications didn't work and the side effects were terrible. So I went after the biochemistry, and after a decade it resolved the anxiety and panic attacks. But it never touched the program underneath. At 20, I had watched my mother die of cancer and watched my father go broke, and that trauma loop was still running. Twenty-seven years later, EMDR reached it and helped reprogram my software.
My patient came in through the psychological door. I came in through the biochemical one. We ended up in the same room. After 30 years in practice, I'm convinced most patients carry both.
The stuck alarm
Whatever starts it, a memory, a trauma, a deficiency, an infection, it converges on sympathetic dominance. The amygdala appraises threat in milliseconds, well before the prefrontal cortex weighs in, while the insula feeds it a constant report from the gut, heart and immune system. There doesn't have to be a lion, only a lion detector that's stuck on. Cortisol should terminate the response, but chronic stress and early trauma drive glucocorticoid receptor resistance. The cortisol is there; the brain can't hear it.
What doesn't happen
The better question is what fight-or-flight turns off. Low vagal tone means less gastric acid, blunted CCK signaling, fewer pancreatic enzymes and poor gallbladder contraction. Without bile there are no micelles, and without micelles EPA and DHA never arrive. The patient didn't fail to take enough fish oil. They failed to absorb it. Acetylcholine itself depends on choline and pantothenic acid from that same lost digestion. You can't just VNS-stimulate a dead battery.
It's a resolution problem. Inflammation should end actively, through specialized pro-resolving mediators (resolvins, protectins, maresins) made from DHA. They shift macrophages from inflammatory M1 to reparative M2. The vagus drives that switch twice: acetylcholine at α7 nicotinic receptors suppresses TNF and IL-6, and in animal models it upregulates 15-lipoxygenase, the enzyme that makes SPMs. Cut the vagus and resolution stalls. The fire isn't bigger. Nobody's calling it off, then with sustained galectin-3 the tissue remodels with fibrosis instead of repair.
The gut-brain axis closes the loop. About 80 percent of vagal fibers are afferent, most carrying gut information. Under sympathetic drive, tight junctions loosen, LPS crosses, secretory IgA drops, butyrate producers and regulatory T cells decline, and mast cells lose their cholinergic brake. All of it travels upward as threat. The damaged gut degrades the very signal that would restore the vagus, and the same inflammatory load is increasingly linked to blood-brain barrier permeability.
The ripple spreads: type 3 deiodinase raises reverse T3 and impairs T4-to-T3 conversion, glymphatic clearance slows, and bile stasis lets mycotoxins and estrogens recirculate.
The missing brake
Glutamate-to-GABA conversion requires P5P. Under stress, transsulfuration burns P5P at two steps, and GAD loses the competition, even without a GAD variant. As SAMe falls and SAH rises, COMT slows and norepinephrine lingers. Every stressor stacks on the last.
What I built because of it
Vagus Nerve Reset, my formula with Nutritional Frontiers, is step one of the reset: quiet the alarm, restore inhibitory tone, and set the table for vagal signaling. Its 20 ingredients map onto the problems above. P5P (100 mg) feeds GAD; lemon balm slows GABA breakdown through GABA-transaminase; taurine acts at GABA-A and glycine receptors; NAC balances glutamate through the cystine-glutamate exchanger. 5-MTHF, hydroxocobalamin, riboflavin-5-phosphate, DMG and choline support methylation by two routes, with niacin as a methyl buffer for patients who react to methylfolate. Riboflavin, niacin and copper support COMT, MAO and aldehyde clearance. Saffron and lithium orotate support amygdala reactivity; choline, pantothenic acid and thiamin supply acetylcholine's building blocks; and copper, P5P and SAMe support histamine clearance through DAO and HNMT. Magnesium is mandatory and dosed separately.
Order of operations
Quiet the sympathetic drive and supply inhibitory tone first. Then the psychological work, EMDR, trauma therapy, breathwork, into a nervous system that can encode it. Then stabilize the gut and vagal afferents, and only then build the cholinergic side. Nerve stimulation works best with that foundation in place.
That's the order my own recovery took. Autonomic state isn't one system among many. It's the gate on all of them. Set the software before you run the restoration programs.
